70 yo female. frequent faint spell.?

70 yo caucasion femaile w/ history of type 2 diabetes. 5′6 144lb. hA1c 9.1, glucose 170, bp 110/70. poor appetite. currently given insulin to control her glucose. Patient complain of frequent fainting spells, back and neck pain, headache, and "bad breath". Differential diagnosis? Treatment, additional workup?

Can you describe these "fainting spells"? Circumstances, the seven attributes of a symptom. You know the drill.
it could be the medicaton she’s on. I took blood pressure meds for a short time and fainted almost every time I tried to stand up too fast…needless to say, they got flushed down the toilet
