bad breath??

i have bad breath and SOMETIMES i can forcely cough out some soft smallish very foul smelling object/particle/substance out of my mouth.I smell it and its dreadfully smelling and i think cause bad breath in me.Can the dentists tell me what it is,what causes it and how to TOTALLY eliminate that smelling substance from my throat?Is it cause by decaying matter by bacteria?And what actually causes it to come,how do i totally eradicate this smelling substance?Please help.

If it is from lack of oral care you need to floss,brush and use a mouth wash several times a day.
